Subsea BOP's are still perhaps one of the or always the top three NPT sources of equipment failure? They may be infrequent but when they occur are still far > 'in accidental' consequential lost time and well costs.
What measures can/are being initiated to > QA/QC of such subsea systems is the discussion issue, where any pertinent comments to mitigate these risk further appreciated.
